The Public Health Digital Library (PHDL) provides access to journals, books, and databases supporting evidence-based practice. Public health organizations pay an annual fee to support access. The National Public Health Coordination Office manages the PHDL program, maintaining the library system, curating the collection, securing access, and providing instruction. This survey is meant to provide each other some basic information. You may not be the person able to make the decision about the PHDL program, that is okay.
We want to ask questions about your organization's size and current access and use of information resources. You will be asked for your name, email, and organization name. Text boxes will allow for your questions, comments, or other details. Except for your contact, there are no required questions; this survey will take about 10 minutes to complete.
